Riddled with cancer and on the verge of death, Amir, the beloved leader of the country of Kalid, confides to his American fiancée, Tracy Wilson, and Dr. Bob Nigserian, his close friend and confidant, that he has decided to undergo a secret, experimental operation in which Dr. Lloyd Trenton will transplant the brain from Amir's corpse into another body. Bob has developed a serum that will prevent Amir's brain from deteriorating for fifteen hours after his death. When Amir dies soon after, Bob injects him with the serum, and with Amir's faithful aide Mohammed, accompanies the body to Trenton's laboratory in Manila. There Trenton assures Bob that no one will ever discover the deception because once Amir's brain is transplanted, plastic surgery will sculpt the new body into the image of Amir. As Trenton's dwarf assistant Dorro assists Trenton with the delicate operation, the doctor's other assistant, the grotesque Gor, stalks a man in which to transplant Amir's brain. The victim, a burglar, sneaks into an apartment through a window, but when the robbery goes awry and the burglar realizes that Gor is following him, he runs onto the roof and falls to his death in the ensuing struggle. When Gor delivers the thief's broken body to Trenton with only moments to spare before Amir's brain will begin to deteriorate, the doctor decides to transplant the brain into the horribly disfigured body of the simple-minded Gor to "buy time." Meanwhile, Bob and Mohammed, who have been dismissed by Trenton, are driving along a deserted road when a car forces them off the road, causing their vehicle to roll down an embankment and burst into flames. Mohammed is killed, but Bob manages to escape and spot his attacker standing on the road above. Upon completing the transplant, Dorro goes to the cellar, where he has imprisoned two women in order to drain their blood for use in Trenton's experiments. After Dorro drags one of the women off, the other, Katherine, discovers that he has left his keys behind and uses them to unlock her manacles. Katherine timidly makes her way through the dark, dank hallways and becomes terrified upon seeing Amir's mutilated body. Soon after, Tracy's plane lands in Manila and she is met by Bob, who informs her that Mohammed is dead and that he suspects someone of sabotage. Meanwhile, the man who forced Bob off the road calls Trenton from a phone booth to report that he has killed both Bob and Mohammed. When the assassin steps out of the phone booth, Bob spots him and pursues him onto a rooftop. After a struggle, the man thrashes Bob and runs to his car, but when he turns on the ignition, the vehicle explodes. Dorro, who had planted the bomb and is watching from nearby, chuckles. Back at the laboratory, Trenton tells Dorro that they will be traveling to another country where he plans to seize control so that he can dedicate all their resources to science. Before Trenton can carry out his plan, however, Bob, accompanied by Tracy, arrives at the laboratory, surprising the doctor, who was led to believe that Bob was dead. When Bob demands to see Amir, Trenton takes him to Gor's slumbering body and awakens him. Bob, shocked at Amir's new, grisly appearance, informs him that Mohammed is dead. Upset, Amir gets off the operating table, and when he sees his reflection in a window, furiously tries to strangle the doctor. Dorro intervenes, however, and convinces Amir to spare the doctor. Tracy then tells Amir that she believes Mohammed was murdered by traitors who want to overthrow the government and accuses Bob of being one of them. When Bob tries to prevent Tracy from leaving with Amir, Amir knocks him out, but upon reaching their car, discovers that Dorro has disabled it, forcing them to continue on foot. Telling Dorro to "take good care" of Bob's body, Trenton, armed with a device that emits waves to trigger an electrode planted in Amir's brain, sets off after them. While shoving Bob's body down the cellar stairs, Dorro spots Katherine. When he tries to restrain her, she seizes a hypodermic needle Dorro has prepared for Bob and plunges it into the dwarf, killing him. Katherine insists on accompanying Bob on his search for Amir, and in the countryside, they come upon a young boy, who says that a monster is following him. Realizing that it must be Amir, Bob instructs Katherine to stay behind with the boy and hands her a pistol to protect herself. As Amir pursues the boy, Trenton, who is secretly in league with Tracy to take over Kalid, catches up to her and shows her the electrode device with which he can control Amir. Just as Amir reaches Katherine and the boy, Trenton shoots him with the electrode device, causing him to run off in pain. Alerted by Katherine's screams, Bob returns to her and confides that he feels complicit in the killings because he approved of Trenton's plan. Katherine kisses Bob and tries to comfort him, but he asks her to take the boy and go home. Crazed and confused, Amir finds Tracy, and when he begins to strangle her, Trenton zaps him with the device and announces that he plans to rule Kalid. Just then, Bob appears and, denouncing their experiment as a travesty, orders Amir to stop. Agitated, Amir allows Tracy to slip out of his grasp, and while fleeing in terror, she blindly plunges over a cliff to her death. Zapping Amir again, Trenton goads him into killing Bob, after which he transplants Amir's brain into Bob's skull and transforms his appearance with plastic surgery. Upon returning to Kalid, Amir announces that he has decided to funnel the country's resources into establishing a great scientific institution that will be headed by Trenton, the new Minister of Science.

The opening and closing cast credits differ slightly in order. The picture was filmed in the Philippines. Although onscreen credits contain a copyright statement for Phil-Am Enterprises, Ltd., the film was not registered for copyright at the time of its release. In 1996,Brain of Blood was registered for copyright by Phil-Am Enterprises, Ltd., at which time it was assigned the number PA-833-817. The copyright records for the 1996 video release of the film listed the alternate titles of The Creature's Revenge, The Brain, Brain Dead and Brain Damage. Modern sources add Mike Haven to the cast.
